Please complete this form if you wish to join AVAHO. Once your form has been submitted and reviewed, you will receive an email confirming your membership and you will be able to pay your dues online. You will need to log back into your account to do this so please remember your email and password you created.

Note: Employees of commercial companies are not eligible to join AVAHO.

We invite any health care professional with interest in the area of hematology/oncology to join AVAHO. This includes, but is not limited to: physicians, scientists, pharmacists, nurses, educators, administrators etc. While AVAHO's focus is the VA employees and patients, membership is not exclusive to VA employees.

Per our bylaws, employees of commercial companies cannot be members of AVAHO.

Membership dues required annually.
