Instructions

Please complete this form entirely if you wish to join AVAHO. Incomplete applications will not be approved. Once your application form has been submitted and dues payment completed, your application will be reviewed to see if you meet our membership criteria. Once your application has been approved, you will receive an email confirming your membership with instructions as to how to set up your membership login (user ID and password).

Once you have created your user ID and password, if you have any issues accessing your AVAHO account, please contact AVAHO's Membership Director, Candy Crawford at candy@avaho.org or 206-708-3033.

AVAHO Member $75 Annually

Membership dues are billed annually and are based on a calendar year (Jan-Dec). Dues must be paid annually regardless of whether or not you take advantage of any of the many benefits that AVAHO offers it's membership.

Should you no longer wish to be a member of AVAHO, you are required to contact the AVAHO Membership Director by email to cancel your membership otherwise your membership will automatically renew each year.

Networking opportunities with other oncology and hematology professionals
Access to quality, low or no cost educational forums and CEU's for licensure requirements
Shared information regarding trends in cancer care

Access to industry to understand trends and issues
The ability to vote and hold office within the AVAHO organization.
Access to early registration for the AVAHO Annual Meeting. This is our 3 day annual event.
Access to regional meeting which are normally a 1 day event with a specialized topic.
Access to AVAHO travel support to attend the Annual Meeting
Access to the AVAHO Mentorship program

We invite any health care professionals with an interest in the area of Hematology/oncology to join AVAHO. This includes, but is not limited to: physicians, scientists, pharmacists, nurses, dietitians, social workers, educators, administrators, etc. While AVAHO's focus is the VA employees and patients membership is not exclusive to VA employees. Per our bylaws, employees of commercial companies cannot be members of AVAHO.

Trainee

Medical students, interns, residents, fellows are eligible to join AVAHO for free.
